Regular Civil Process:
For initial Complaint these are the required documents: (See sample forms below)
1. Complaint Please bring original signed form and 2 copies; if more than one defendant, then an additional copy is needed for each additional defendant. It is helpful if the original is signed with blue ink.
2. Summons (20 day) Please bring original and same number of copies as the Complaint (above).
3. Praecipe-Instructions for Service Only original, signed copy needed. It is the Plaintiff's responsibility to choose who they want to serve the Summons. See List of Process Servers.
4. Praecipe-Instructions to Clerk This form is optional. Only needed when instructions to court are unusual.
